聲明及責任

謹此聲明本人參加 『大年初一利是跑 2024』一切有關活動，本人願意遵守由大會為該活動不時訂立的所有條文及規則，並同意以下各點：

1. 本人是自願參加活動和願意承擔自身意外風險及責任，並無權向大會對本人在往返活動場地途中、活動中發生或引致之自身意外、死亡或任何的損失索償或責任；

2. 本人聲明本人身體健康並有能力參加此活動；

3. 本人願意接受大會要求及提供的藥物治療及檢查；

4. 本人明白同意提供本人的香港身份證或護照予大會，以作核對個人資料；

5. 本人提交此報名表格表示同意及確認所有列明之重要事項、聲明及有關細則；

6. 本人聲明報名表格上資料確實無誤；

7. 本人願意授權大會在無須要徵得本人同意而可以使用本人肖像、姓名、成績及聲音成為大會活動推廣及宣傳使用；

8. 本人同意接受大會有關活動的宣傳資料及訊息；

9. 所有個人資料將絶對保密，並會嚴格遵照法律規定處理；

I hereby declare that I have participated in all relevant activities of the 『 Red Packet Run 2024 』, and I am willing to abide by all provisions, and agree to the following rules:

1. I am willing to participate in the event and willing to bear my own accidental risks and responsibilities, and I have no right to claim or be liable to the event organiser for my own accident, death, or any loss that occurs or is caused by me during the competition or from the event;

2. I declare that I am healthy and capable of participating in this activity;

3. I am willing to accept the medical treatment and checking requested and provided by the organiser;

4. I agree to provide my Hong Kong identity card or passport to the organiser for personal data verification;

5. I sign this registration form to signify my agreement and confirmation of all important matters, declarations and related details;

6. I declare that the information on this registration form is correct;

7. I am willing to authorize the conference to use my portrait, name, and voice for the promotion of the event without requiring my consent;

8. I agree to accept promotional materials and messages related to the event;

9. All personal data will be kept strictly confidential and processed in strict accordance with legal requirements.
